Project Description: The Florida Department of Financial Services (DFS), Division of Workers’ Compensation (DWC), Bureau of Monitoring and Audit is issuing this Statement of Work (SOW) to define the scope and requirements of this task order pursuant to the State Term Contract for IT Consulting Services (STC), Project Area 4: Staff Augmentation. This support is needed because the Division does not have staff to perform routine maintenance and support of the Centralized Performance System (CPS).
Scope of Work:
The scope of work for this project may be generally described as analyzing, designing, programming, testing, training, and implementing releases for the Centralized Performance System’s enhancements and maintenance, and assisting in ensuring the application’s integrity and stability. CPS is an interactive web-based system that gives insurers and claims handling entities the ability to review and respond to their performance information in real-time. The system has three components: a medical module, an indemnity module, and an audit module.
The .NET part of the CPS application (the CPS front end/middle tier) consists of approximately:
· 200 web pages, 55 user controls, 150 vb.net code files, 45
· Assuming an average of 275 lines of code per web page, CPS has approximately 55,000 lines of code
The database tier for CPS consists of approximately:
· 45 packages, 141 tables, 300 stored procedures, 79 views
2.2 The tasks to be performed by the Contractor include the following:
· Report to the Bureau Chief of Monitoring and Audit or Section Manager assigned to the project
· Provide project management when assignment is defined as a project according to standards defined in DFS’s Project Management Office
· Develop components of CPS application in regards to enhancements and/or errors.
· Systems analysis
· Systems design
· Systems programming
· Testing/Quality Assurance (QA)
· System Implementation
· Upgrade support for CPS application
· Crystal Reporting support for CPS
· Technical documentation creation and update maintenance as required using DFS templates where appropriate as identified by the Contract Manager.
· DIS infrastructure changes application support
Deliverables and Acceptance Criteria.
The Contractor agrees to provide all services and deliverables pursuant to the Customer requirements identified in this SOW. (Hourly rates are stated in the Pricing Proposal.)
Deliverable: 4.1 System analysis design and programming support, and/or implementation support for applications
Acceptance Criteria / Performance Standards For Each Task
- Provide application and technical documentation as required by Customer
- Project Management documentation as required by the Contract Manager.
- Fully tested and commented application code implemented in the production environment
- Transfer of knowledge to Customer’s staff regarding the CPS system, including technical knowledge related to the system, determined to be acceptable by the Customer
- Other deliverables as determined by the Customer
- Failure to use the appropriate technical requirements and proper testing to ensure that the appropriate software and hardware are selected and function properly in production will result in task rejection and may not be invoiced or paid until correction of the task
- Task must be satisfactorily completed within the stated response time stated in DFS’s project plan or the deliverable review schedule per SOW Sec.5, after request is made, and must be itemized in invoice
4.2 Application & Technical Documentation
- Work performed meets established DIS standards for a software development process (ISDM)
- Documentation complies with the DIS policies, procedures and technical standards provided by the Contract Manager
- Deliverable review process for documentation has been completed by the Customer and resulted in sign-off
- Knowledge transfer to maintain system
- Task must be satisfactorily completed within the stated response time stated in DFS’s project plan or the Deliverable Review Schedule per SOW Sec.5, after request is made, and must be itemized in invoice
4.3 Test Plan, Suites/Scheduling and Results Log
- Describes test suites, test conditions and test cycles that must be performed to ensure the system is thoroughly tested
- Scheduling for testing, identifies the requirements, impacted users, testers, developers, start and end dates
- Log test results
4.4 Status reports
- A report of services provided by the Vendor attesting to the level of services provided, and listing the frequency of responses and confirmation that response time was met.
- Day-to-day issues are tracked and resolved
- Report status of action items and resolution
4.5 Time Reports
- A time report must be submitted showing the projects/activities worked on each day by each contracted staff, along with the hours spent on each project/activity and the remaining balance of hours for each contracted staff
- Failure to complete the required duties as outlined in the SOW may result in the rejection of the invoice
- Must be included and submitted with invoice monthly
Submission of Resumes
· Date(s) of availability, hours of availability (hours per week available to work).
· Resumes must show employment history for all relevant and related experience and all education and degrees.
· A background check including fingerprinting will be completed on the consultant working on this project prior to the contractor commencing work
· Must submit pre-screening reference checks for candidate submitted
· Must supply a matrix of candidate skills highlighting years of experience with each required skills using the skills matrix below.
